23 Family Care Teams of the 35 recommended by Health Accord NL have been announced.
Announced in 2022: St. John’s West, St. John’s Centre, St. John’s East, Southern Avalon, Stephenville-Bay St. George, Coast of Bays, Northern Peninsula, and Happy Valley-Goose Bay area.
Announced in 2023-2024: Clarenville, Conception Bay North, Metro Centre in St. John’s, Deer Lake-White Bay, Port aux Basques, Labrador West, Gander, Grand Falls-Windsor, Kittiwake (Brookfield and area), Bonavista, and Burin Peninsula.
Announced in 2024-2025: Baie Verte and Springdale region, Lewisporte, Portugal Cove-St. Philip’s, and Downtown St. John’s.
What is a Family Care Team?
Health care isn’t one size fits all. Every person (and community) is unique, and therefore we need to take a needs based approach to planning teams and an individualized approach to care. That’s why we introduced Family Care Teams.
At a Family Care Team, a team of health professionals, including family physicians, registered nurses, nurse practitioners and other health care providers (such as physiotherapists and dietitians) come together to deliver care that meets your individual health and social needs.
How does a Family Care Team support me?
A Family Care Team will work with you, and other providers on the team as needed, to support all your health and social needs. The team works together, with one centralized health record, to provide the best care possible to you and your family.
The goal is to connect you with the right care, in the right place and at the right time. Depending on your reason(s) for accessing care from the team, you might be best served meeting with a physician, nurse practitioner, or another provider like a social worker, diabetes nurse educator or an occupational therapist. That’s what a Family Care Team offers – a team-based approach to meeting your health care needs.

How do I find a Family Care Team?
Family Care Teams are at various stages of development in many areas across the province. If you do not have a primary care provider now, you can register with Patient Connect today so Newfoundland and Labrador Health Services can assign you to a provider or a Family Care Team when one becomes available in your area. The information collected through Patient Connect is also helping us plan for more Family Care Teams.
